Description:

 The Care Coordinator for IHM (Integrated Health Management) members of the QUEST population is responsible for supporting the member by:
General Competencies
  • Expediting access for members to services
  • Rapid response to provider/member needs in community based settings and inpatient levels of care.
  • Ensure continuity and integration of services between all parties
  • Work collaboratively and collegially within an integrated model
  • 3-5 years basic level of competency and knowledge of Behavioral Health Industry Knowledge, Care Coordination, & Progressive experience in technical environments

Responsibilities:

Responsibilities 1. Coordinates, with oversight by Case Managers, all information and referral functions from those calls including initiation of care plans to ensure continuity and integration of services.Provide continuum of quality care through telephonic and field based outreach, education, crisis intervention and other clinically based activities to plan members as specified in their treatment plans. Conducts assessments, assist with the development of case management and crisis plans for medical treatment and/or behavioral modification within the scope of practice.2. Maintains logs and other data bases regarding case management, utilization review, and care coordination activities of assigned caseload.3. Track aftercare outpatient appointments following inpatient or acute levels of care of assigned caseload.4. Perform on-site care coordination during medical office hours as needed.5. Assist members in accessing care by educating providers, members and providing/arranging transportation as necessary.6. Identify individual limitations, deficits, and strengths and aggressively attempt to provide patients with what they need in relation to medical, behavioral and social needs.7. Help reduce the negative consequences to the individual when there is a lack of follow up and participation in treatment.
8. Participate in daily rounds from provider sites, when applicable.
9. Participate in monthly meetings to discuss feedback from providers/members re: our continuum of care. Additionally, assist in identifying gaps in covered services.
10. Obtain signed releases of information from members via on site collaboration with providers.11. Facilitate joint rounds with medical case management to assist in identification and outreach with medically compromised members.12. Other duties as assigned.

Bachelor’s degree
